SMS Health Assessment Program
The health pass program is a health attestation of health service designed for businesses to protect their employees during a pandemic through a screening program. This screening program is used by businesses to text their employees and screen them for possible COVID-19 symptoms. Based on the employee’s answers, Affirmed Healthy Service will automatically text the end-user a health pass which they can show to security in order to gain access to your building. The health pass will contain the person’s name, their health status, the current date as well as a QR code that can be scanned by security for verification and tracking. You can also use this service to ask employees and visitors to send you pictures of their body temperatures daily for further protection. This health pass service enabled on 30482 enables businesses to do the following:
- Send COVID-19 symptom-related questions to their employees and/or visitors
- Receive MMS messages from employees and/or visitors
- Send recurring SMS messages to employees daily
- Create custom health questions
- Customize health passes
- Send personalized images to employees and visitors
The SMS health assessment screening service on 30482 (the “Program”) is provided by Affirmed Healthy (“We,” “Us,” “Our”, “Affirmed Healthy”) as the Program Sponsor, and enabled by Skycore LLC. Skycore LLC is incorporated in the Commonwealth of Massachusetts with offices at 397 Moody St. Suite 202 Waltham MA 02453.

By opting into the Program or otherwise sharing your personal information in connection with Affirmed Healthy, you expressly consent to the applicable Terms of Service listed below. In the event of any inconsistency between the provisions of these Mobile Terms and Conditions and the Terms of Service below, the Terms of Service shall govern and control.

The service allows users to receive SMS/MMS text messages from 30482 by affirmatively opting into the Program or requesting information from the Program by texting the designated keyword to 30482. Regardless of the opt-in method utilized to use the service, you agree that these Terms apply to your participation in the Program. By texting an opt-in keyword related to this Program or replying affirmatively to a message from this Program requesting the user’s consent to receive recurring messages, you are agreeing to these Terms and are agreeing to receive recurring text messages. You must be 18 or older or have permission from a parent or guardian to participate.
Cost and Frequency:
There are no additional fees or premium charges to receive text messages, however, standard carrier message and data rates may apply. The Program involves one time messages as well as recurring messages. The frequency of recurring text messages may vary based on the plan that the customer chooses.
To opt-out of receiving text messages from this Program, text, or reply “STOP” to 30482. You will receive an additional SMS message confirming your decision to opt-out of this Program. At all times you must clearly communicate their intent to unsubscribe from the Program by utilizing opt-out commands. Additional supported opt-out commands can be sent include “QUIT”, “END”, “CANCEL”, “UNSUBSCRIBE”, or “STOP ALL”.
We will not be liable for any delays or failures in the receipt of any SMS/MMS text messages. Delivery or timeliness of SMS/MMS messages is not guaranteed and subject to the sender or recipient’s service, connectivity, mobile device, and wireless service provider/network. T-Mobile is not liable for delayed or undelivered mobile messages. SMS and MMS messaging content may not be compatible with all phone models or carriers.
You warrant and represent that you have all necessary rights, power, and authority to agree to these Terms and perform your obligations hereunder, and nothing contained in this Agreement or in the performance of such obligations will place you in breach of any other contract or obligation. The failure of either party to exercise in any respect any right provided for herein will not be deemed a waiver of any further rights hereunder. If any provision of these Terms is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary so that this Agreement will otherwise remain in full force and effect and enforceable. We reserve the right to change these Terms from time to time and you acknowledge your responsibility to review these Terms from time to time to be aware of any such changes. Your continued participation in the service after any such change shall constitute your acceptance of the modified Terms. If you do not agree to a change in these Terms, you must immediately opt-out using the commands listed above.
